Emergency Medical Consent
In the event of an emergency requiring medical attention for my child, I understand that Summit School of Chess instructors will make every effort to contact me. However, if I cannot be reached, I give permission for a licensed physician or medical treatment center to administer emergency medical care if needed.

CORONAVIRUS/COVID ACTIVITY PARTICIPATION ASSUMPTION OF RISK, RELEASE, WAIVER, AND DISCHARGE
COVID has been declared a worldwide pandemic by the World Health Organization. COVID is extremely contagious and is believed to spread mainly from person-to-person contact. It is believed that an asymptomatic individual can be infected with and transmit COVID without their knowledge. Chess Camp of the Rockies. has put in place preventative measures recommended by the State of Colorado to reduce the spread of COVID, however, Chess Camp of the Rockies cannot guarantee that you, your child(ren), your spouse, or anyone else will not become exposed to or infected with COVID as a result of participating in the Chess Camp. Participation in the Chess Camp of the Rockies could increase the risk of contracting COVID. NOW, THEREFORE, in consideration of being permitted to participate in the Chess Camp of the Rockies, I understand, acknowledge, and agree to the following: I have independently evaluated and reviewed the risks of being exposed to or infected with COVID and have determined to participate in the Chess Camp of the Rockies with full knowledge and acceptance of the risk. Fully understanding these risks, I, for myself, my child(ren), my spouse, my legal representatives, heirs, and assigns, hereby agree to assume full responsibility and liability for the risk of bodily injury, illness, permanent disability, and/or death which may result from exposure to or infection with COVID before, during, or after participating in the Chess Camp of the Rockies. I, for myself, my child(ren), my spouse, my legal representatives, heirs and assigns, hereby waive, release, covenant not to sue, discharge, and hold harmless Summit School of Chess, Master Mind Chess, Chess Camp of the Rockies, its instructors, supervisors, any agents and/or employees as well as other camp participants and their families of and from the Claims including any and all liability, claims, actions, damages, costs or expenses of any kind to me, my child(ren), my spouse, my legal representatives, heirs, and assigns, for any and all losses or damages resulting from bodily injury, illness, permanent disability, and/or death, whether caused by negligence of Chess Camp of the Rockies, its instructors, supervisors, any agents and/or employees as well as other camp participants and their families or otherwise, which claims, losses, and demands arise during or result directly or indirectly from exposure to or infection with COVID before, during, or after participating in the Chess Camp of the Rockies.
By providing this signature and participating in, or authorizing participation in, this Summit School of Chess camp in any way whatsoever, you hereby release and hold harmless forever Summit School of Chess, its instructors, employees, agents, and consultants from any and all liability, for any claim, demand, injury, expense, damage, action or cause of action arising out of or connected with the services or facilities of these entities, including those arising from acts of active or passive negligence. All participants, guardians and observers must comply with Perl Mack Community Center’s code of conduct.
Indemnification and Waiver of Liability
By registering for any program, the registrant acknowledges that program activities carry certain risks for participants. Further, by registering, the registrant releases and indemnifies Summit School of Chess and Chess Camp of the Rockies, its instructors, supervisors, any agents and/or employees as well as other camp participants and their families from and against any and all claims, demands, loss or injury to person or property caused during participation in the activity. This release and indemnity is intended to be as broad as permitted by law.
